Applying for a card

If you attend an independent school, a college or school outside Derbyshire, you're educated at home, under 17 but no longer in full time education, you missed the b_line photography sessions in your school, or you need an urgent replacement card during scholl/college holidays this is the way to get your card.

Get a card large

You will need to fill in an application form (opens in a new window) and send it back to us with a passport-sized photograph. There are three ways to get hold of your form.

  1. Download a copy of the application form, print it off, fill it in, get it checked and send it to us with a passport-sized photograph. Your b_line1 or b_line2 card will be sent direct to your home address within three weeks.

  2. Call 08 456 058 058 (8am to 8pm weekdays) and an application form will be sent to you.

  3. You can also pick up a form at your local library or from your school.          

Remember if you have lost or damaged your card you'll have to pay a £5 replacement charge (sent in with your completed application form, ticking the box for lost or damaged cards). You will also need to send a new photograph with your application.
