Lost or destroyed your card? Didn't automatically receive one? Then this is the page for you.

Whatever the reason, this is what you need to do:

  1. Fill in an application form:

    • download a copy from this page

    • call 08 456 058 058 and we will send you one

    • pick one up from your school.

  2. Get a passport sized photo done - don't use your school photo - we're not allowed to use these.

  3. If your card's lost or stolen, you'll need to include a cheque for £5 with your form. This just covers the cost of printing a new one. Remember to tick the box for "Lost or damaged cards".

Application form

There are two types of application form, the one you need depends on where you go to school and a few other things. You can use the list below to work out which form you need.

Standard Form

  • You will be getting your application checked and signed at a mainstream school or college within Derbyshire
  • Exceptions/Other info
  • Your form and photo must be taken to the main reception/admin office for signing - not to teachers/tutors/lecturers, as this will delay your application!
  • You can complete the standard form if you attend any of the following schools outside of Derbyshire - Aquinas College, Ashby School, Ashton Sixth Form College, Bilborough College, Blessed Robert Sutton School, Burton College, Castle College, Nottingham, Cheadle and Marple sixth form College, George Spencer School, Nottingham, Longendale Community Language College, NCN - City Broadway only, Stockport College, Tameside College, Wales High School, West Notts College – Mansfield.

BX Form

  • You are no longer in education and have not reached your 17th birthday yet
  • or
  • You attend 12 or more hours of supervised study at school, college or a training provider
  • or
  • You are educated at home and don't attend a school
  • or
  • You need an urgent replacement card during school/college holidays
  • or
  • You attend a school or college outside of Derbyshire (see left for a few exceptions)
  • Important
  • When filling in a BX form, please make sure that you get three signatures from the person verifying your form or we will have to send it back to you: On the back of the photo, on part C of the form and on the proof of age document.
