Application form

There are two types of b_line application form, so before you apply please check that your are filling in the correct form.

Most of you will need to fill in the standard application form (please see the related documents).

This form is for b_line1 and b_line2 applicants who will be getting their application checked and signed at a mainstream school or college within Derbyshire.

Your form and photo must be taken to the main reception/admin office for signing - not to teachers/tutors/lecturers as this will delay your application being processed and your form may be returned to you.

If you attend Derby College or South East Derbyshire College you should complete the standard form except for Part C, then take it to your student services reception.

If you are not at a mainstream Derbyshire secondary school or college you will need to complete application form BX (please see the related documents).

Your form, photo and photocopied ID document (photo ID if possible) must be signed by a verifier who can be contacted easily by phone.

This form is for b-line2 applicants who:

  • are no longer in education and have not reached their 17th birthday

  • attend E2E or similar courses through YMCA, Age Concern or other training provider

  • are home educated

  • need an urgent replacement card during school/college holidays

  • attend a school or college outside of Derbyshire, unless you attend one of the following where you may use a standard form (see note re signing of standard forms above):

    • Aquinas College

    • Audenshaw School

    • Bilborough College

    • Blessed Robert Sutton School

    • Burton College

    • Castle College, Nottingham

    • Cheadle and Marple sixth form College

    • Longendale Community Language College

    • NCN - City Broadway only

    • West Notts College - Ashfield

    • West Notts College - Mansfield
